Appointment Request

We are happy to book an appointment for your pet! To request an appointment, click on the "Request an Appointment" button and it will take you to our appointment request form.

You may use the form to schedule an appointment for an exam, or a routine surgery – such as a spay, neuter, or dental. This is not an automatic process and does not guarantee an immediate appointment. Once we receive your request, we will contact you directly to confirm a date and time.

If your pet has an emergency or needs immediate care, call our office asap.

Currently, we are scheduling annual and routine exams approximately for 1 to 2 weeks from the submission of this form. If we have not confirmed your appointment request within 24-48 hours, please feel free to contact the clinic by phone to confirm at (360)871-3335.

Refill Request

Please click "Request a Refill" below to request your prescription refill or food item for pick-up at Woodside Animal Hospital. (This is not our online store for home delivery.) You may use this form to refill products in the clinic such as Bravecto, Carprofen or other items.

Please allow 24-48 hours for order processing. Your order will not be ready for pick-up until you receive a confirmation from us. Many prescriptions require your pet to be examined before refilling or dispensing medications. This ensures that the medication is appropriate for your pet’s current condition. If we have not confirmed your refill request within 24-48 hours, please feel free to contact the clinic by phone to confirm at (360)871-3335.

IMPORTANT: Prescription refills and food orders are not confirmed until you have received notification. A staff member will contact you by phone or email.
